Recent VITAL study report: Not so Vital Statistics after all

A recent VITamin-D & OmegaA-3s TriaL or the VITAL study report, published in ‘Clinical Chemistry’ revealed that neither Vitamin-D nor the Omega-3s supplement is effective when it comes to reducing systematic inflammation. It is an ongoing study comprising 25,871 men and women in the US and was commenced in July 2010 at the Brigham & Women’s Hospital in Boston, Massachusetts. This recent result was published in the Clinical Chemistry journal on November 7, 2019.

Even though Vitamin-D and Omega-3s fatty acids from marine sources are claimed to be beneficial in reducing systematic inflammation, the results of the study do not favor. For those who would confuse as to what systematic inflammation is, its symptoms include, various cardiovascular diseases, heart attacks/failures, diabetes mellitus, osteoporosis, and even cancers and neuro-destructive diseases such as Alzheimer’s.

The objective of VITAL study

The VITAL study aims at investigating if the daily intake of Vitamin-D or Omega-3s supplements helps to reduce the risk of systematic inflammation or cancer, heart diseases, etc. among people who do not have a prior history of such ailments. They mainly include men above 50 years of age and women above 55. Due to the lack of any large-scale clinical trials or studies, doctors have faced a dilemma while prescribing Vitamin-D and/or supplements like the Omega-3s to their patents. With a large-scale VITAL study, it becomes easier to determine whether it is the right medication or not. However, the test results remain unsatisfactory.

The beginnings of VITAL Trials

You may be thinking of how the VITAL trial commences or works? Well, as I said earlier, it is an investigation comprising of 25,871 men and women across the US with a prior history of systematic inflammation symptoms. These men and women are treated with a daily dosage of the said supplement, and the effects are recorded and analyzed. In more clinical terms, it is basically a randomized & double-blind, placebo-controlled trial with a daily dosage of Vitamin-D (2000 IU/day) or Omega-3s supplement (1 gm/day) or even both. In this study, researchers studied the levels of three of the known biomarkers of systematic inflammation from the start of the study or trial up to a period of one year. These three biomarkers are the IL-6 or interleukin-6, hsCRP, or the high sensitivity C-reactive protein and the tumor necrosis factor receptor two or TNFR2.

Reports show that even after a year of taking these vitamins and supplements, there is no decrease in the levels of these three biomarkers. Surprisingly enough, an intake of the Vitamin-D supplement raised the IL-6 levels by 8.2 percent. However, investigators do report that a decline in hsCRP was seen in those using the Omega-3s supplement with lower fish intake during the early stages of the study. It must be noted here, that this recent study was done among a relatively smaller population (1500) in comparison to the original population of approx. 25,000. However, this is a carefully selected sample that can accurately represent the entire population.

Some of the baseline characteristics considered for the study other than gender include ethnicity, geographic location of residence, highest levels of education, a self-reported annual income of less than &50,000 pa, smoking, alcohol intake, and more. Other than the cardiovascular ailments and cancer, this study also aims to find the effect of these supplements in reducing the risks of a few other diseases like:

⇒ Diabetes mellitus;

⇒ Cognitive decline (memory loss);

⇒ Physical disability;

⇒ Anemia;

⇒ Various infections;

⇒ Depression;

⇒ Asthma;

⇒ High blood pressure;

⇒ Diabetes-related kidney disorders;

⇒ Rheumatoid arthritis;

⇒ Chronic knee pains;

⇒ Bone fractures;

⇒ Macular degeneration & dry eye syndrome and

⇒ Thyroid disease.

Study groups of VITAL

Related Vitamin-D studies have proved fruitful while discerning its effect in reducing some of the syndromes sited above like, chronic knee pains, depressions, and infections, etc. However, these results are not definitive due to various errors in selecting the samples from the VITAL population. The VITAL study has four major study groups, namely:

  1. Vitamin-D & fish oil;
  2. Vitamin-D & fish oil placebo;
  3. Vitamin-D placebo & fish oil and
  4. Vitamin-D placebo & fish oil placebo.

The last one being a total placebo group is not considered as the “real” study group, where every person has a 25% chance to be placed in anyone among the four. Some study findings are cited below:

⇒ 12,944 participants were incorporated into the Vitamin-D placebo group (gr. 3&4) of which 824 citations of cancer was found;

⇒ 12,927 participants were included in the Vitamin-D group (gr. 1&2) of which 793 citations of cancer was found; this is a small but non-insignificant improvement;

⇒ Placebo group cited 409 major causes of cardiovascular diseases while the same was 396 in the count for the Vitamin-D group;

Vitamin-D supplements remain ineffective in reducing breast, colorectal, or prostate cancer; although there has been a suggestive reduction in cancer-related deaths by about 17%; with 23% among African-Americans;

⇒ 419 events of cardiovascular disease were reported from the 12,938 strong groups of Omega-3s placebo participants (gr. 2&4);

⇒ Among the 12,933 participants in the Omega-3s group (gr. 1&3) 386 major causes of cardiovascular disease were cited; an 8% reduction which is not yet of value as per the researchers;

⇒ Seven hundred ninety-seven cases of cancer in the placebo group were reported while the same was 820 for the fatty acid group, once again, an insignificant difference.

Thus one can conclude from these results that neither Vitamin-D nor Omega-3s and their supplements are effective in reducing systematic inflammation and cancer and CVD. However, the study did conclude one thing: people with lower fish intake during the start of the program do have a chance of reduction in one of the biomarkers. Future VITAL analyses will thus be something most of us would be keenly interested in. Researchers involved are focusing on the ones that address the reduction in risks of diseases and not their biomarkers.

Is Dementia Related to Trans Fats? Most of the Time, It’s a Yes

Researchers undertook a study comprising of old adults in a Japanese town. During the study, it was found out that people with high levels of trans fat in their blood are more likely to be at risk of developing dementia than their counterparts with either lower or normal levels.

Can dementia be related to trans fat?

It seems like the answer is yes – and a recent study proves just that.

During the study, the biggest culprit turned out to be sweet pastries – they were the most dominant source in people with high levels of trans fats in their blood.

As far as trans fats are concerned, there are two known primary sources in the human diet – artificial and natural. While the sources of natural fats are known to be dairy products and in the meat of some animals, the sources of artificial fats are considered to be the primary ones in the human diet, and which makes vegetable oil solid by adding hydrogen to it.

In the words of the American Heart Association, it is the consumption of these trans fats that raise the risk of developing heart disease and stroke and has even been linked to developing signs of type 2 diabetes.

According to a recent study published in the online paper of the American Academy of Neurology, it was mentioned by the authors how they had linked trans fats in people to a higher risk of developing dementia by taking the help of data that had been taken from a group of people living in Hisayama, Japan.

Toshiharu Ninomiya is one of the authors from the above-mentioned paper, He is a professor of Kyushu University in Japan, and a renowned name in the field of epidemiology and public health. He has mentioned that the results they have found from the study, it gives solid evidence of why people need to avoid trans fats and even though. If people tend to take trans fats in small amounts in food, over time, they will add up if people take multiple servings of the same food, because trans fats are still allowed in many countries.

55 – 75% of a higher chance of dementia

According to the study, it gathered the data of 1,628 people who were either 60 years of age or near it, and without any signs of dementia. Their blood samples were taken for a screening test in the year 2002-2003, and then the researchers followed them for ten years to see if they had developed any notable signs of dementia.

During these ten years, out of 1,628 participants, 377 of them had been diagnosed with early signs of dementia, and 247 of them for Alzheimer’s disease.

To get the results from the study, the researchers divided the group of participants into four equal groups based on their blood levels of trans fats.

The group that had the highest percentage of blood levels of trans fats – their incidence rate of developing dementia was 29.8 per 1000 person-years, with the next group being 27.6 and so on.

The researchers also pointed out that various other factors can lead to developing signs of dementia, such as diabetes, high blood pressure, and smoking habits.

Finally, the researchers concluded that the group that had the highest percentage of trans fats in their blood, they have a 52% higher chance of developing early signs of dementia, with the second group having a 74% chance of developing dementia.

The United States bans trans fats.

In 2018, the Food and Drug Administration banned trans fats in the United States, claiming that this step will help to prevent hundreds and thousands of heart attacks every year as it means removing partially hydrogenated oils from processed foods.

However, it is to be noted that the ban allowed some foods to pass and to say that they contain 0 g of trans fats even if they contain 0.5 g of trans fats – and which means that some food will consist of small amounts of trans fats.

The researchers also stated that many studies had been conducted previously to investigate the link between dementia and trans fats, but they had been inaccurate – it was mainly due to that earlier, there was no suitable method for assessing the intake of trans fats in the human diet.

And now, for this study, the researchers mentioned they had taken the help of the blood levels of elaidic acid. It is a major trans fats that can be found in partially hydrogenated vegetable oils, and which would act as an objective biomarker for industrial trans fats.

Sweet pastries are linked to high trans fats.

As part of the screening test, the group of participants was asked to fill in questionnaires on their food intake, and by gathering all of the data, the researchers were able to point out which foods contribute to high levels of blood trans fats.

According to the authors, sweet pastries contribute to the highest percentage, with croissants and sugar confectionaries taking the second and third places, respectively.

Nowadays, even rice crackers and ice cream can be included to the list, they have added.

The researchers also stated that since the data for the study is taken only from a single town in Japan, so, they are not sure as to how the study will fare with data taken from the other countries, and whether the results will be the same or not.

But as the United Nations health agency has put it, the results may give varied results from other countries, but it won’t be that much different since trans fats cause more than 600,000 deaths per year due to cardiovascular diseases worldwide.

According to Prof. Ninomiya, the WHO has released a step by step guide on how one can remove trans fats from the food chain, and furthermore, it’s good news that WHO has taken steps to eliminate trans fats by the year 2023.

Whether it be an antagonist like in Goldeneye or an antihero like in Psycho, the concept of evil has remained the same until the genre of drama started to address a different form of evil.

This evil entity did not have arms or legs or even ahead, but it tingled with the very functioning of the human mind. These were mental illnesses. Alfred Hitchcock famously said, “Drama is life, with the boring bits cut out” and conventional filmmakers thought the delicacy of a human mind to be the boring bit. But since the 80s, a wave of serial killings and suicides were on the run. People like Ted Bundy or John Wayne Gacy fascinated people with the possibility of a human mind comprehending the parameters of humanly possible and transcending them. While the drill instructor appreciates the individuality in Private joker by making him the captain of the squad, he makes a massive mistake. Not only the drill instructor fails to make him a perfect killing machine, but he fails to serve his basic task as a soldier when he is sent to Vietnam. The film deals with an on-field crisis that the troops faced, the duality of the human mind, and asks the very important question, is it worth to go through all the torment the troops go through?

The Breakfast Club:

Rain Man:

Man is a 1988 movie directed by Barry Levinson, starring Tom Cruise and Dustin Hoffman. This humorous classic follows the greedy automobile dealer Charlie who kidnaps his autistic brother Raymond when he discovers that he is going to inherit all of their late father’s belongings. The premise of the movie is the mistreatment of autistic people by conventional people. Charlie’s greed takes over his love for his brother, but during their time together, he learns that Raymond has a very interesting take on life that he delivers in every punchy line. Charlie also discovers that his brother has an extraordinary memory and remarkable math skills. The film is very prominent in presenting autism to the general audience of the late 80s.

A Clockwork Orange:

Kubrick classic takes a different tone on mental health. The 1971 film is an adaptation of the classic novel of the same name by Anthony Burgess. Our protagonist is Alex, a psychotic delinquent who, along with his gang, which he calls Droogs, commits acts of violence like theft, rape, and finally murder which accompanied by a betrayal by his gang, lands him in prison. Alex, who wants to get out of prison, accepts to participate in a government experiment for a reduction in his sentence. In the experiment, he is forcefully shown violent imagery with music that makes him susceptible to violence. He becomes nauseated, even with the slightest hint of violence. When he comes home, he is mistreated by everyone around him. A Clockwork Orange does justice to the portrayal of Antisocial Personality disorder. A well-crafted antihero like Alex presents the old disease in a new world. The society bluntly plays in the development of his psychosis. The film depicts the treatment of the disease that fails miserably, which acts as a metaphor for the common belief of effective treatment of a mental treatment that can only make things worse for the sufferer.

Nevertheless, solo dining can affect differently to people of different sex, age group, and ethnic orientation. 

 

The largest promoters of the solo dining traditions are the major fast-food chains. Why waste time sitting and enjoying a meal with your closed one when you can have a quick bite of your own and get back to work? The whole tradition is built upon a corporate infrastructure, and we can all agree that the food industry is the largest business in the world. However, the methods used by fast-food chains cannot be deemed as sustainable. The growing promotion of mono-culturalism is killing the planet from its core and profiting the industry in billions. A sustainable food future that will be eco-friendly should be undeniably replete with instances of sharing food with others. World Mental Health Day – What’s The Need

Indeed, mental illnesses have always been stigmatized throughout the world for the longest time and to date. It was never accepted as a normal sign of breakdown but a phenomenon that could make the sufferer an abnormal person. Sufferers endured their disability, perceiving the fact that they would not be considered ‘normal’ and treated in the most indifferent way. But today, the world, along with evolution in the diverse genre, has excelled in the field of taking care of mental health and preventing people’s demise too.

This year, the World Federation for Mental Health or WFMH, celebrated this day by theming it as suicide prevention. According to the World Health Organization or WHO, it is more than 800,000 people passing away by committing suicide, with the majority age group being fifteen to twenty-nine years. Further, it has been discovered that not only adults are victimized, but young ones too are preyed upon by violence, sexual abuse, bullying, cyberbullying, and more. The intensity felt by those tender minds are so emotionally overwhelming that they find suicide the only way possible to hide the pain and make an end to it. This particular affair has never been brought to light so explicitly before, but this year the WFMH has turned out to be the game-changer.

The fight against the suicide

 

With around 8,00,000 sufferers committing suicide, it is narrowed down that in just 40 seconds, somebody is committing it. Not only this, when that one person is committing it, 20 others are attempting it. New invention towards the victory on blindness

Fight against blindness is a big challenge in the field of Science and Technology. A recent report of the world health organization says that around 1.3 billion people live with some visibility impairment. Out of which, 39 million Peoples are completely lost the vision. In European and American country, visual acuity 20/200 and the visual field below 200 is considered as a parameter for blindness.

Visual acuity 20/200 means, a person will able to see an object in such a magnification as a normal person see at a distance of 200 meters. As per the report, the distressing thing is that in most of the cases eye could be saved if care and proper medication were provided to the patient. The life of a person with impaired vision is intricate. They lack education and often socially discriminated. One can observe their problem in transport or during accessing buildings etc.

Encouraging report

A recent report in Nature Biomedical Engineering Journal is considered as a new hope for people who have blindness or extremely low vision. To discuss it, let us first focus on some basic points related to the eye. Blindness is a result of any damage or alteration in the vision-related pathway, i.e., cornea, lens, retina, optic nerves, thalamus, and visual cortex. The cornea is the eye’s outer most lens that helps to focus and control incoming light; it bends and refracts the incoming light on the lens. Lens again further focuses the light taking help of vitreous humor- a transparent gel-like substance that also provides support and attachment to retina. Image forms on the retina and further retina converts the optical signal into an electrical signal, which finally carried through optic nerves up to the thalamus and visual cortex.

Thalamus is the part of the brain which helps in relaying of sensory signal. The visual cortex is connected to the thalamus, and it processes the visual signal such that a person can exactly imagine the objects having the correct shape, size, and distance of the object. Problematic cornea distorts image, cloudiness, and in more severe conditions, causes blindness. Breakdown of a small portion of the retina in the case of age-related macular degeneration, hereditary disease degenerative retina, or retinitis pigmentosa is also retina related disorder. Damage of bundle of the optic nerve due to inflammation and pain is common, and hence, there is disruption in the signal going to the brain.

Now let us come on recent new development reported in Nature Biomedical Engineering Journal. Scientists report this work belongs to EPFL Switzerland and SSSA Italy. The novelty of this work is that it bypasses the complete pathway of the eyeball and directly sends the signal to the brain. So all the complication related to different pathway of eyeball does not interfere any way in this new technology. They invented a new thin intramural electrode which directly stimulates the optic nerve. This new invention has already been tested successfully in a rabbit. This new technology uses the concept of phosphine. Phosphine can be understood as a phenomenon in which one can experience seeing the light without actually any entrance of the light in the eye. Phosphine is generated directly by stimulating the visual cortex. In the present invention, prosthetic device implantation is required to do. The prosthetic device is made of medical-grade elastomer material already been popular as implant materials. Elastomer is further coated with a polymeric material known as PDMS to save the parent material for any degradation after implant. Two electrodes, cathode, and anode, acts as a generator and make the phosphine movable.

Bionic Eye

Now technology is reached in a stage where the complete eyeball is being neglected for seeing an object. But earlier first remarkable experimentation, namely retinal visual prosthesis (bionic eye), was reported in the year 2014 in which a group of Scientists from Centre of eye research Australia and other six Australian organizations reported this system. It was successfully tested in three persons of different age groups. In this bionic eye cathode made of platinum grown on silicon substrate was used, two return electrodes were also required in this system. However, it has produced some positive visibility results, but the main problem was related to the large size of electrodes and its energy back up. However, testing this device on the first three implanted persons was carried out for two years. In clinical test bacteria, S Aureus infection due to implantation was observed, and it raised a question mark in the success of this system.

Argus II

The Second remarkable invention of an ongoing clinical trial is reported on the Argus II Retinal Prosthetic system. Five years of clinical testing results were published in 2016. However, this trial is being carried out for ten years, and still, the clinical trial is continuing. Argus II is a little complicated system having glasses attached with a small camera that further connected with a video processing unit. Implanting material has an array of 60 electrodes and a sealed electronic amplifying circuitry containing an antenna too. When the system is switched on, then camera collected video signals are processed, and real-time data is sent by an outside antenna to the receiving antenna inside the eye. This external device is kept fixed on the body of the patient by using a strap. In this clinical trial, 30 people of different age groups are participating, however, this system seems a little complicated, but it is exhibiting encouraging results.

Argus II was granted for trial by European regulatory in 2011 and the USA in 2013. Longest10 years of the trial reported being full of encouraging results. By 2021 expected to come in the market for the use of general masses. Out of 30 trial patients, few have only shown complication.15 out of 30 implants are continue to show reliability, long term efficiency, and fulfilling safety norms. However, two devices have shown failure, but the related technical problem was corrected in time. In one case, chronic hypotony was reported. Experts are correlating the problem with the patient’s other diseases developed before or during the trial period. The longest trail in the field of eye implant is Argus II, and it is indicating success and the nearest possibility for availability for masses.

Silicon retina

In the year 2010, an artificial silicon retina was proposed, and it was implanted in six patients suffering from retinitis pigmentosa for a clinical trial. In this system, a photosensitive selenium cell and a microchip were also being implanted. This was initially exhibited good results as the visual acuity, perception of color, darkness, etc. were good enough. But after two months of trial, a sudden problem in vision was observed, and finally, the vision was lost. The problem with electrical stimulation was detected, and finally, this attempt did not materialize.

Ultrasonic method

Ultrasonic wave for retinal stimulation was first introduced in 2012. The principle behind this is related to the transmission of real-time camera captured video signals through acoustic ultrasonic waves. The ultrasonic signal is seen quite capable of exciting retinal neurons. The good thing with this method is that it does not require an inside implant. Only a multi-element phased array is required to wear near cornea, infect it require to paste there. In ultrasonic stimulation, many questions are still to be answered by the scientific community. Long term effect or tolerability of ultrasonic wave is one of the questions. Scientists are working on a different technical issue. After a successful trial report, this device can come into the public domain.

Infrared neural stimulation

In 2005 use of infrared laser pulses to give neural stimulation was reported. Tissues water content get the infrared wave adsorbed. Adsorption results in photothermal stimulation. Water absorption of the infrared wave concept is used for visual restoration in this method. Black micro particle photo absorber is being used in this experimentation. Now conjugated polymer is replaced as a photo absorber. Work in different laboratories is going on in this visual restoration method. Biophysical and physiological characterization of this method is still in the testing period. This product has not come for trial so far.

Availability of Biocompatible implant materials

In this field, one of the major challenges is related to the availability of proper eye implant material. In most of the above-mentioned cases, the failure of experimentation was either related to the incompatibility of implant material or electronic devices. Once an implant is done in a patient the toxic effect of the implant with the period is another problem. In many earlier tests or trials, it has been found that initially, the implant worked well, but with the time, it started showing some infection or non-functioning kind of problem.

In the present time, a lot of work on graphene, carbon nanotube, or conjugated polymer is going on as an eye implant material. Reports coming are very encouraging. One of the main issues in the field of materials science is related to the reproducibility of fabricated material. But thanks God, new technology on 3 D printing of material has provided an amazing relief. In 3 D printing technology replica of material with full physical, chemical, and mechanical property can be fabricated. 3 D technology can be assumed a remarkable turn in the field of materials engineering, whatever be the complication in designing the material, one can do it in CAD programming and can be fabricated as per design using 3 D printing technology. This technology is cost-effective also, hence patients have not to worry much about implant costs too.

Dry Eye: This problem is related to not proper secretion of tears or due to poor quality of tear. Applying warm compresses gives relief in this disease. Giving a short break to the eye is helpful. The habit of blinking properly is also one of the ways to prevent this disease.

Allergic Conjunctivitis: If a person is careful especially in the day which has a high pollen level then conjunctivitis is not going to affect. A chemical known as histamine present in pollen is the cause of this disease.

Glaucoma: Glaucoma causes due to high eye blood pressure. The disease can be prevented by regular exercise; exercise keep the pressure under control.

Tips to keep eye vision healthy

Foods: Fish such as salmon, tuna, etc are rich in Omega 3 fatty acids, it saves the eye from cataract, dry eye syndrome, and macular degeneration. Nuts are also rich in Omega 3 fatty acids. Citrus fruits are rich in vitamin C; it helps in reducing any kind of inflammation in the eye. It helps in maintaining optic nerves. Leafy vegetable contains carotenoids such as lutein and zeaxantnan helpful in maintaining retina health. Carrot is rich in vitamin A, helps in maintaining the ocular surface of the eye.

Specific exercises: Relaxing our eye in our long continues work is important. One simple exercise one can do by rubbing the palm for a few seconds and keep the palm on the eyelid. Let the warm transfer to the eye. This can be repeated twice or thrice. Blinking of the eye for 10-15 times very fast also provides a lot of comfort to our work stress eye. This should also be repeated for 4-5 times after a short interval.

The third effective one is sit somewhere comfortably and bring your both thumb together close to your eye by bending your arms. This a process of zooming your thumb. Rotating eyeball clockwise and anti-clockwise regularly at least once in a day improves the eyesight.

What to avoid: Smokers more likely to get into cataract problems. Smoking damages the optic nerves. Keep away looking direct sunlight or any high-intensity light source, it damages the retina. Long hours of continuous television watching is also not advisable. Proper Eye protection from dust, air pollution, snow, UV ray, etc is essential.
